THE FOLLOWING FORMS ARE USED IF A TITLE IS LOST.
APPLICATION FOR DUPLICATE

Completed by the registered owner of the manufactured/mobile home if there is no
lien shown on the title record. Completed by the lienholder if there is an existing lien
shown on title that has not been paid off.
BILL OF SALE

Completed if title is lost and the home is being sold by the registered owner
shown on title.
LIEN SATISFIED

Completed by lienholder shown on title if title is lost and lienholder has been
paid off.
